Course Policies & Student Agreement

Medical

All student divers, for each course taken, are required to read and complete the Diver Medical Form. The purpose of this form is to find out if you should consult with your doctor before participating in recreational diver training. A “Yes” response to a question does not necessarily disqualify you from diving. A “Yes” response means that there is a preexisting condition that may affect your safety while diving and you must seek the advice of a physician prior to engaging in scuba diving activities.

You must answer each question on your past or present medical history with a YES or NO. If you are not sure, answer YES. If you can answer YES to question 3, 5, or 10, OR any question on page 2 of the form, thenwe mus t request that you consult with a physician prior to participating in scuba diving.

Your physician’s impression regarding your medical fitness for scuba diving must be documented on the Divers Medical Form. If your physician finds no medical conditions considered incompatible with diving, without restrictions or conditions, you must provide the completed form to your instructor at the start of your first course session, and prior to any in-water training. Student Divers who are professional physicians may not sign their own medical form. If your physician is unable to recommend you for diving, you must provide the completed form to the staff at Scuba Frisco at least 2 days prior to the start date of your course.

Should you experience a medical condition or concern during your training which would change your answer(s) on the Divers Medical Form and/or affects your ability to safely scuba dive, then your instructor will be required to suspend your training. In order to resume training, you will need to complete a new Divers Medical Form and obtain physician approval to continue your training. Your comfort, safety and well-being while participating in scuba diving activities is our primary concern.

Certification

Our instructors and divemasters will do their very best to provide a positive learning environment, answer your questions completely, assist you with challenges, and ensure you meet all training requirements necessary for certification. However, PADI instructors are under no obligation to issue you a scuba certification and there is no guarantee that you will be certified. Training requirements are performance based and the instructor must be sure that you have completed all training modules according to PADI standards, are comfortable in the water and can perform all diving skills repeatedly, with confidence, and without assistance (except for skills where assistance from a dive buddy is allowed or required).

Should you not be able to meet the course performance requirements in the scheduled time frame, we will continue to work with you to achieve your scuba certification. Additional charges will apply for additional training sessions beyond those listed on your course schedule.
